You may build a form from scratch by creating a blank form and adding all desired form elements.
Note: It is highly recommended that you duplicate an existing form when creating a new form. For more information, see the Duplicating a Form section of this documentation.
- Select Form Setup from the More dropdown menu.
- Navigate to the Event Forms domain.
- Click the Form view button.
- Click the Form Setup tab.
- Click the New Form button to create a new, blank form.
- Fill in the empty fields in the Form Setup tab.
- Add the desired Form Elements.
- Click the blue Upload Form button to commit your changes.
